Is it stain you are trying to get rid of or odor? Once a dog has soiled on a carpet we can sometimes get the odor out so that we cannot smell it but the dog usually can and it is the smell of urine that attracts them to urinate some more in the same area. If the urine has gotten into the under pad there is not much you can do to stop the dog from soiling more except to watch it constantly and get it outside in time.
I think at that point I would take up the carpeting and put down some kind of hard floor surface.
If you have used a professional steam floor cleaning machine then there is nothing better than that to clean a carpet.
