A dog not on monthly parasite prevention could indeed have worms. One flea swallowed is all it takes. I almost hope this is the issue, because that is an easy fix.

Yes maggots can go into their skin, there are members here who had to have maggots removed from their dogs.

Call the vet as soon as you can. An exterminator too.
