  If a dog is on frontline, it will prevent fleas from biting the skin.  It will not prevent the dog from ingesting a flea - that's the cause of tape worms.I would ask your vet about a fecal and if you should do a course of drontal.  A regular dewormer wont always cover tapeworms.   
I know you are saying it's not near their poop or them but I find it suspect that you are only seeing the worms in the potty patch -- are they anywhere else?  The potty patch if washed regularly as you are doing and have been for 1.5 years seems to not be an environment to cultivate worms, especially if you are promptly picking up feces.
