If mine put up that kind of fight, they would be muzzled and restrained or scruffed and restrained. Your restrainer should be able to hold this way. The other option is wrapping your pup like a burrito.
If you can't do it, the vet will have to. That could possibly mean a few techs holding him down while muzzled. Whatever you decide, just try to get your pup over this because it will only get worse if not deal with and vet staff aren't going to be all warm and fuzzy if that is going on at the office...
