Hi. Welcome. I really think you should consider either taking your dog back to the vet you already saw or getting a second opinion. A scraping of the ear for a specimen probably needs done to identify the source of the problem to properly medicate - bacterial source, yeast source, fungal source? Each different possibility requires a different course of treatment/medication. Especially if the puppy is scratching so much there is scabbing and bleeding, this really should be addressed. I'm sure this is painful to the little one.
