  Watch closely and see if it always connected to outdoors.  If it is, I bet your pup is nibbling a little grass (mine do it and hack away; lots of Yorkies act like sheep and eat grass for no reason), or it is probably an allergic reaction to something outside.

  The really hard heaving doesn't sound good.  Both of my boys went through about 2 weeks of reverse sneezing this spring, right after Max had surgery.  With him, I'm sure anesthesia and intubation were factors.  I think both the boys were reacting to the dampness and humidity and mold or whatever else was growing outside.  Teddy even did a thing that sounded like a human clearing his throat, but it usually was tied to drinking water too fast.   
If it continues, I would try videotaping it for your vet to see and hear.  Sorry I don't have any better info or suggestions.
