Have you had anything unusual going on, like guests in the house or travel? Changes of any kind?

Be sure to give praise for correct potty habits. When she goes potty where she is supposed to, praise profusely and give a small treat (I use kibble). Never stop giving praise. I would also limit her freedom until her habits improve. Keep her in the same room with you. Essentially start over with potty training if it continues.

You may also want to try another cleaner. Could be that you are not getting the scent out of the spots. Try Nature's Miracle and a carpet cleaning machine.
