I am so sorry your baby is not doing well. As a breeder myself, I would be very concerned about his size. Has the vet considered that he may have a liver shunt? Often puppies with liver shunts do not thrive and they have blood sugar problems. What kind of food did you wean him on?
If you have been giving him a lot of Nutri-cal then his pancreas might be exhausted. Please read the following article:

I realize your little one is an older puppy but this solution might help him. The first thing is to get him stable. If you keep giving him different types of food then you might be doing more harm than good. It could be upsetting his tummy. I recommend you get a good quality dry puppy food and grind it up. You can make a mush using 1/2 goats milk mixed with 1/2 water. Or in place to the goat milk you can use some canned Pedigree Puppy food mixed with the ground up dry food.
