I'd like to believe that it's just the ear, but with the excessive water intake added in, I'd be at the vet (well, probably another vet for a second or third opinion) today doing some bloodwork at the very least. Basic bloodwork and a bile acids test really should be done asap. If it continues, you will also need to see a neurologist if the bloodwork is okay.

You may be able to think of this as the preop bloodwork before getting your pup altered. As long as it's done in the near future, you could just repeat the CBC right before surgery (if you are planning on altering).
