It could also be she's really excited to see other dogs and people. While on a leash, they can't happily, and quietly approach another dog they see in the distance. They can try, but get to the end of the lead and no go anywhere. It's almost like they're saying, "hey over here! I wanna say hi but your kinda far!!! Hi hi hi!"
What you could do is recruit some friends or neighbors to he'll you train. While walking with her, approach the other dog or person. As soon as you see her attempt to run towards the dog of person, or try to bark, immediately turn around and walk in the other direction. When you have her focused back on you and she is calm, turn around and try again. With repetition, she'll learn that being calm gets her to where she wants to go instead of pulling and barking.
