OMG, this is horrible! What did the vet say? This poor little kiddo.
It's hard to say what this is - but since he is a puppy, mange/mites def needs to be ruled out. This does not look like allergies to me. From what I can see, it really doesn't look bacterial either, but that could be determined by an easy, quick skin scrape.
I'm glad you found him, please get him to the vet asap!
