Default My 3 year old can't stop itching

My yorkie has the itch and it led to some hair loss, and I have had him at the vet a couple of times, and he gave him some steriod shots & antibiotics which only temporarily fixed the problem. The vet said he was allergic to fleas. That is something I had great difficulty getting rid of until recently, and it still seems my yard treatment wasn't fully effective, so my dogs can carry some back in. But right now, no fleas on any of my animals. My yorkie still wants to eat himself up. I have to keep an outfit on him so he doesn't do this, but he will take his teeth and bite and scratch fist size blood spots on him. His hair has been dry, I've noticed white flakes in it, he lost hair again, so I just gave him a good shave the other day, and left his facial hair long. Still, wants to eat himself up. I have started rubbing cocoa butter on him for his dryness, currently have him on another antibiotic. Please please please tell me what to do for this little turk. I am not sure my vet properly diagnosed his condition, when he is still itching that badly with no fleas. I have tried hydrocordisone shampoo too and no luck. I have also had him on allergy medication I bought from a pet shop. Should I put him back on this medication?
