If she has had him since he was a pup and has not taken him to the vet, he has not had his 1-year booster shots. The law in most states requires rabies every 3 years, so she will eventually have to go.
Overall, I believe it is bad. A dog should be examined once a year by a doctor and should have a fecal and heartworm test done, at the very least.
