There's actually a pretty straighforward chart of what is rare, med rare, medium, medium well, and well done. Some people just don't know how to follow it.

Rare: cool, red center still 75% red
Med rare: warm, red center still 50% red
Medium: pink center, 0% red
Medium well: Brown 75%
Well done: brown 100% (this should not be called "well done" as a steak cooked to this point is certainly not a job well done LOL )
