Goat's milk has been mentioned here. It should be clarified that the goat's milk is the canned stuff you buy in the baking aisle (next to the condensed milk) and needs to be diluted 1:1 with water and have a small amount of karo syrup added to it.

She may not drink a lot, but there is quite a bit of water content in canned food that will help keep her hydrated. Adding some formula/goats milk to it will help, too.

At 5 weeks, she will be prone to episodes of hypoglycemia. Nutrical is a must have, followed by an easily digestible protein such as boiled skinless chicken breast to help prevent rebound hypoglycemia. Keep meal times frequent for now (at least 4 times a day). You can also leave some dry kibble out for her 24/7.

As cute and tiny as she is, she's going to need lots of rest. Keep play/handling to an absolute minimum right now (no more than 10-20 minutes max).

Also, her immune system is very weak at this point. No trips outside (use potty pads), no trips to petsmart (or any other place where she can come in contact with other dogs/germs). Even at the vet, keep her off the floor. She won't be considered fully immune until about 2 weeks after her last set of puppy shots.
