Throw a party when she poops on the pad. Go wild, without scaring her. We have so much fun celebrating good potties in our house that my boys wouldn't miss the opportunity to do it in the right place.

Perhaps try a different cleaner or two to eliminate the scent in the two spots where she is continuing to potty. If you catch her in the act, I agree with Cathy, redirect her to the correct area, calmly.

When you can't watch her, limit her roaming area/freedom to one room with the pad. Always make sure that the pad is easily accessible. If she has to travel across the house or from one floor to another, she might not be inclined to make the trip.
