DHPP is the basic puppy vaccine and should be given alone. CV is Corona Virus Vaccine and is sometimes mixed in with the DHPP, but that is not recommended (too much for a small pup), and Corona Virus only affects pups under 6 weeks of age, so he doesn't need that anymore.

At 14 weeks he can get his 3rd DHPP, if he is well. (1 month inbetween vaccines)

Depending on what your state law is, Rabies is usually given between 4-8 months. With a tiny pup, I would wait as long as possible (8 months), just keep him inside or in your arms until 2 weeks after he gets it. (if healthy)

At 1 year old, he needs a DHPP Booster, then one month later, the Rabies Booster. (if healthy)

I can't tell you what to do... but I have stated what I would do. I am not a Vet, my post reflects my opinions, and this vaccine schedule should be discussed with your vet. If your vet disagrees with this schedule... I would seek a second opinion from a different vet. Vaccines are very important, and as overvaccinating can be dangerous, it's important to know about them.
