I used to think my dog would never tell me when he needed to go out. But, after probably about 8 months, I realized he actually was...I was just missing the signal....he would come up to me and make a snort/sneeze sound....the problem is that he would do the same thing when he wanted to play too! I thought he was just being demanding (he would fetch his ball non-stop, all day long if he could) and I didn't want to acknowledge this for fear of reinforcing a negative behavior.

Somehow I eventually put two and two together and realized that the snort-sneeze noise had multiple meanings! So, maybe your dog is giving you a signal he needs to go out and you don't recognize it, because he's not letting you know in the way you want or expect.
