One question: How are you giving the Milk Thistle? Liquid in a bottle usually contains a lot of alcohol. I think that Marin is normally given with Denosyl and Sam-E, and are all good for the liver.

Also, I believe you've been seeing the vet, as L/D is a prescription diet. I would get her on the Lactulose and Marin along with the Denosyl and Sam-E that she already takes, and then access her mental abilities at that point. Lactulose works quickly so you'll know in a few days if toxins in her blood is causing her problems.

If you don't see a change in her behavior, I would keep her on all the supplements, but then see the vet again about problems that affect older dogs, as there are meds to help with those conditions (such as CCD: Canine Cognitive Disorder) also.

Proteins from dairy and plant sources are easier on the liver, like cottage cheese. If you are considering home-cooking for her, Olga on the DLD (link above) can make dietary recommendations, or you could see a vet nutritionist.
