If your vet is concerned about doing it, look for another vet who has alot of experience with toy breeds.
He'll be fine, I always suggest bloodwork before any anestesia first though. Nueters really don't take very long so it's not a major surgery, the anesthesia is the biggest part, but if bloodwork comes back normal and the vet is used to administering it to tiny dogs, he should do fine.
