Steroids injections can work very well. If he has to wear a cone 75% of the time, it sounds like a major quality of life issue.
Have you done a true elimination diet with a novel protein and carb (or hydrolyzed protein) for at least 12 weeks with no other food given?
Antihistamines aren't great for allergies in dogs. They work sometimes for environmental, but definitely not always. If the elim. diet didn't work, has allergy testing been done (for environmental allergies)? Atopica tried (or allergy shots)?
It sounds like there is an immediate need for relief. I'd personally be doing steroids until a diagnosis and better treatment plan can be figured out. However, you can't really do allergy testing until steroids have cleared the system.
