What you need to do, is you need to get him to a vet for skin testing and blood testing.

Since he is getting worse, he needs to see a vet ASAP. It's fine that it isn't fleas, however, there is a ton of other skin-related issues that this could be.

The fact that he is losing fur and his skin is now red, means he could be also have an infection (all over). He's miserable, from what you're saying; he needs you to take him in for help.
