It sounds like you have a real mess on your hands! Looks like you are doing a really good job with her all except this problem. I agree about taking her poo out doors and putting where you expect her to do her business. I would also try paper training her. Be sure to place several layers of news papers near the door that you use on regular basis. When she has an accident putting the poo on the papers will show her where you want her to go when training in the house. But your ultimate goal being Out Side Only! Paper training will get you one step closer to that goal.
