How old is Elvis? If he's still a young puppy the biting your fingers is just playing, but you are right for discouraging it. Just tell him "no" like you've been doing, but then give him a toy to chew on and play with instead. Even the little growl when he goes for your fingers is just playing if he's a puppy, that's how he played with his littermates.

As far as coming out of the crate, Alice doesn't like to be "taken" out of her crate that she sleeps in at night either. The first week or so I had her, she wouldn't walk out of it either, I had to reach in and get her, but now I just open the door and out she comes Idk why he comes out for strangers, maybe he's just more curious of them. Try using a small treat to get him to come out on his own, he should get over that soon
