Could it just have been that the drug hadn't gotten to it yet?
Heartworm disease is possible, but I wouldn't think it would be too far advanced with no symptoms, yearly testing, and year-round preventative.
Some drugs work better than others.
If adults are present, there is still an extremely good chance that everything will be fine with treatment.
