Poor little guy. My brother's dog did something similar to that, and when they put him under to do his dental, he ended up having a 4in thorn stuck in his gum line.
However, I wonder if he possibly choked on one of his teeth and then got it down.
But I'm not a vet, and I think you need to get to a new vet asap for preventative measures.
