Leaving the head still in the dog isn't that terrible. Sure its not ideal but its not the end of the world. The body will just treat it like a foreign object and either reject it or destroy it (by the white blood cells). a red bump may arise but that should go away. Just be sure to clean the affected area with alcohol or peroxide, and don't forget to wash your hands as well.
