Have you thought about a second opinion?
I don't want to say what is right or wrong b/c I really am not 100% sure. Here though, I was watching spays one day and one of them came up positive for demodex (thinking maybe this was a recurring thing for this pup so they were rechecking). The vet said after the spay that she probably should have held off (already weakened immune system - could make surgery recovery a bit more challenging, but also the stress from the surgery is not going to be good for the immune system).
I'd find a vet that I trust to make the decision. If I was deciding on my own, I'd wait (probably need more specialized care anyway because this sounds like a bad case).
