Hi...
One thing I see is you let her out, she pees on the pad, but you go to set your phone down. Try letting her out of the crate, let her pee on the pad, then take her right outside to poop. Have treats in your hand before you first let her out of the crate. This has to be a fluid set of actions on your part, as she knows how to take advantage of your distractions (walking away to put the phone down). Reward her for peeing on the pad, and you need to catch her pooping in an appropriate place so you can reward her for that too. Ignore her accidents. I know how hard that can be, but they 'get it' so much quicker when we can be positive and happy with them.
