Etsy.com has some AWESOME tag makers! They can personalize them for you. Check it out.

My ultimate favorite tags are Fetching Tags but they are a bit pricey. I have two and love them, but no other info besides name and number and a little symbol will fit on these.

Red Dingo also makes really cute sturdy tags.

I put Jackson's name on his tags because it honestly wouldn't matter if someone saw his name, and knew his name, he most likely would not run up happily to a stranger anyways, lol. I think the main reasoning for people doing this is so a stranger can't call your dogs name and act like they know the dog. I've heard not to put microchip #'s on tags because you don't want a thief to take the chip out of the dog themselves if they know it's chipped, or never take them to the vet if they are afraid the dog will get scanned, etc. So I prefer to keep that # off his tags.

I keep his rabies tag and his AKC ID tag on my keychain actually. That way I always have them with them.
