It could be dermatitis or his thyroid...I would go to the vet and have them check...My girl Bailey RIP...she was about 8 years old and started scratching and smelling...she had no fleas...so I took her to the vet and he checked her thyroid, her thyroid had stopped working...once we started the synthroid and it got in her system good she was back to normal..she had to take it daily for the rest of her sweet life.
