My 17-lb cavalier threw up repeatedly about 9 months ago, and was shaking. We took him to the emergency vet (!) and they said if we hadn't brought him in he would have probably died. And he's 17 LBS! It was acute pancreatitis and he spent 2 days in emergency care, costing me about $1500.00. It was a scary, touch and go situation, and we reacted immediately.
If you are too busy to care for a small dog that is as sick as you say, then there is a problem in addition to the dog's illness.
