Well first congratulations on the new little one.

My thoughts are
- Try a Kong small stuffed with frozen peanut butter or chez whiz
- Try placing a t shirt of yours in the kennel with her.
-Try Being calm for 20 minutes before you go and leave that picture in her head of calm. Do not go any where near her till you have been home 20 minutes. Making it all normal as you can.
- Try practice leaving so get her in the crate click and treat. open the door.
Then in the crate and half way to the door.
Do everything you do before you go and do not leave, then just go in and out the door then to the car, then to start the car, then out the drive way and back again.
Yes it will take a long time but it will work and not all of this in one day just little parts and will take a while to get to actually leave stress free. At any point she gets upset stop at that step, or go back to the step she was ok at and repeat till she can move to the next step calmly.
- Lastly there is a med that can take the edge off if these steps do not work as the stress level is to high but you Will need to talk to your vet and not it is not Prozac. Depending on if you are in the states or Canada it was a different name and I can give the name to you if the time comes you need it

She needs time to settle in and understand that you are for good and not for just right now. She was had her trust shattered and it will take time and a ton of love and patiences to get it.
