I am sorry for your friend's tragic loss. We use the Buddy Belt harness and leash. I don't worry about my boys getting out of the harness, but I have nightmares that the leash hook will break when they pull hard to get at the garbage truck or UPS truck. Because my boys are pullers in these situations, I won't use Flexi leashes or any other kind of retractable leash. I stick to ordinary nylon leashes.
