I'm a visual person too, and when I was in school I kept a notebook of footnote, endnote, and bibliography examples to use as a key for all of my papers. There are so many little details about citations, I couldn't possibly keep them all in my little head.

I also looked at footnotes and bibliographies in books in my collection.
For
Ibid., I would do a separate line for each one, even if you do it 4-5 times in a row. Keep in mind that you don't need to footnote each of your sentences. Use your judgment where to place the footnote. If you have a paragraph or two pulling facts or ideas from a single source, footnote at the end of that section. For a quote, I would footnote at the end of the quote or sentence containing the quote.
The style Lil Fu Fu Girl referenced is good too. I believe it is a matter of preference.
For the bibliography, include one full citation for each source.
Ibid should be italicized with a period, I believe, but here is an example (scroll down):
