First, I have to disagree that 11 months is late to housebreak a dog. Look at the poll that some genius (that would be me - ha! ha!) posted:

Although the sample size is small, you'll see that about 35% of our Yorkies were either housetrained at that age or older OR they still aren't housebroken at that age or more.
So, don't give up! You are not alone.
A little more information would be helpful. Would you tell us how you react when Lexi 1) potties on the floor and 2) potties on the pee pads?
Also, for curiosity, how big is Lexi? And, when you talked to the vets, did they check for a UTI (urinary tract infection)?
