I don't know anything about heat as I've never experienced one with any of my pets, but I've done reading. It sounds like she could be in heat as I believe that's ONE sign, but there should be others. Hopefully someone who knows more will chime in.

I DO know that going through heat can cause increased risks of certain cancers, etc., but I'm not sure how much one cycle increases those risks. Again, hopefully someone else can chime in.

I understand why you waited with her being so small, but I've always spayed prior to first heat... so unfortunately I don't have any insight for you. I wish I could help more!
