If you trust your vet, and he's not starving for money, he or she will usually do what's necessary without going overboard. I just got back with Ruger after his checkup, and the doc didn't want to do a lot of the tests I asked about, including heartworm. Just said they weren't necessary. He also looks hard at the risk/reward thing.
