Look around at some breeder and/ or rescue applications and see if you can get some ideas for questions. You will want to know who lives in the house- kids, etc. If the people have ever owned dogs before and what happened to them. What they plan to do with the dog when they are at work and how long the dog will be alone each day. What will the people do with the dog if he/she pees on the floor, ruins something, etc. If they are prepared to exercise the dog daily. Are they prepared for vet bills, grooming bills, etc. Ask for a vet reference and groomer reference if they already own a pet- if not ask them to research local vets and groomers and to have a list of ones they are going to try.
