How much is she scratching? She needs a true allergy diet. Bil Jac isn't what you want. The vet can prescribe one or you could homecook. This is actually what I do. The recipe should be formulated by a veterinary nutritionist. Of course it will only help if it's a food allergy. Do you have old receipts with the names of meds you have tried? You can't use Synotic with other drugs in the ear. It's a steroid and I would highly recommend that you ask your vet about it. I've used Otomax and Mometamax and Zymox for yeast. Otomax is my favorite followed by Zymox. If you need to keep giving something for a bacterial or yeast infection (Synotic isn't for this), then you can give steroids at the same time. Has your vet tried that? Temaril-P has worked well for us as do short term injections of Kenalog.
