There is no such and such form for parti color yorkies. You just put the numbers 014 in the color spot. then send pics with the registration form so they have a record of where the spots are on the dog for identification purposes.
Sometimes the pup does not have enough white to be a parti so AKC papers will say black and tan with white markings or blue and tan with white markings.
