It needs to stay on whenever you can't be right by her to watch (so it can come off while she is eating if you're right there). Alternatively, baby onesies do work well for some pups (but some chew their stitches through them). It's always best to check with the vet before using one though because some don't like the cloth rubbing on the incision. This is what we did for Ellie's spay because I didn't want her in an e-collar for two weeks.
