For now remove the rug by front door, and use a Enyzmatic shampooer that removes all the scent of the dog pee.
Do the same for your kitchen. For puppy pad training limit her space. Take her to her pee pads, after eating, getting up from sleeping, and every two hours, reward her richly for going potty, and when you see her go, use the command go potty, or something similar.
Toe biting, umm harder. Get yourself a long feather or something soft, and everytime she goes for your toes, sharp no, wave the wand, and then if she ceases and desists immediately good girl. Then give chew toy. they are ravenous for chewing as pups, so have lots of good chew toys around.
