That's part of what you budget for before you get even the FIRST dog. The cost of exams, vaccinations, preventive medications and sterilization need to be taken in to consideration and set aside before the dog is even brought home. Too many people spend all their money on the dog (or go for a cheap dog from a mill/byb because they 'can't afford the outrageous prices of yorkies' ) without adequate planning. It's been said over and over again, but the cheapest part of owning a dog is the dog it's self.
