i'm sorry you were both scared. that can happen if the collar isn't fitted properly. it happened to my cat when she was 8 weeks old and i couldn't find a collar small enough for her.
I always keep a collar on my yorkie for identification purposes. I make sure i can get easily fit one finger between the collar and her neck. I also check to see if the collar can in any way slip over her ears or chin.
when you read about collar fitting rules its typical to see them say you should be able to fit 2 fingers under the collar, but that rule only applies to large, medium, and some small dogs. it doesn't apply to toy breeds.
