When I picked my puppy out and paid my deposit, my breeder had this form she filled out. Kind of like a contract. She asked me to bring it back when I picked the puppy up and I did. This paper had a place right on it for her to record type of shots and date and also type of wormer used and when. it also had type of food used and stuff like that and when I picked him up, I took this paper to the vet with me the next day when I had my well puppy check and he recorded it in his files.
