Liver shunt can't be diagnised with blood testing (can only be somewhat certain). Diagnostic imaging is needed to confirm. If a single shunt outside the liver is present, surgery is usually recommended. U/S is sometimes not recommended because it is very difficult to find shunts this way. They are about 60-80% accurate with a skilled sonography. Nevertheless, they are used extensively and may work for your pup.
I'd be concerned that he is getting worse. Since your vet thinks it's a shunt, I'd go to an internal medicine specialist as soon as possible.
