It sounds to me like she is very young and was taken from her mother before she was socialized and taught proper puppy etiquette..
Dogs do not understand human language, you need to speak to them in their language. Start by using a low growl, if she doesn't respond to that then snap at her with a quick pinch to the neck and making a snapping sound like her mother would.
Time outs make no sense to dogs. By the time you put them in the crate they have no clue why you are doing. The correction has to be instantanious.
