I don't think you're doing anything wrong! Dogs learn best when they are with us. If you have a puppy, my opinion is that you should expect to have lots of accidents for that first year. It just comes with the territory. Use lots of positive reinforcement when she goes outside, but don't sweat the accidents. They will diminish over time.

Personally, I just don't agree with the whole crating thing. I have never crated a dog. It just seems so unnatural. Dogs are social creatures and they need to be with us, to interact with us. Sometimes I think the easiest way to "train" a dog is to just spend a lot of time around him/her. The more time they spend with us, the more they understand what our expectations are of them. I spend a lot of time with my dogs and that time has been the best training tool. Vinnie has learned so many "commands" from me simply from being with me. Remember, your presence is more reinforcing than any treat.
