Hi and Welcome to YT!!! The dematologist suggestion is really good. It was an ordeal with our Maggie's allergies that even led to an unnecessary ear surgery until she was seen by a dermatologist to figure out that she has about a gzillion allergies.
It sounds strange to me though that your baby is not itchy b/c the skin irritation comes mostly not from the desease but from the biting or scratching. To do something that might help a bit is putting a clean cotton shirt on him every day. I hope you'll find out soon what's going on!
Oh... and I know about small town vet's! Our vet here is a butcher and filthy so we drive 30 miles to a better one...
